What to expect, what to bring, and where to fill in forms ahead of time.
Our forms are on our patient portal, not on this website. A relative can fill them in with you, or you can arrive 20 minutes early and we will help. We can read the forms to you.

What happens

Step 1: Call
Call the office, or ask a relative to. Tell us if you need a longer time, a wheelchair space, or large-print forms.

Step 2: Forms
Fill them in ahead of time, or at the office.

Step 3: The visit
The low vision exam, with the doctor explaining each step.

Step 4: Before you leave
A copy of your prescription, a written plan in large print, a written estimate for anything you pay yourself, and the next visit.
